Transitions of Western Illinois Fund


In 1954 a group of concerned and visionary community leaders met to plan the development of a community mental health center – an agency that would meet the mental health needs of individuals, children and families in Adams County. In 1955 the Adams County Mental Health Center opened and in 1995 changed its name to Transitions of Western Illinois. From the beginning, Transitions has provided an array of services to help people of all ages and all abilities. 
 
Transitions serves infants and toddlers who have, or are at risk for developmental delays, children who have social/emotional/behavioral problems, and adolescents. In addition, Transitions serves people who have a developmental disability and/or a serious mental illness. Many people who utilize Transitions’ services are people who function well in life until a life event (such as the loss of a loved one, unemployment, or interpersonal or marital problems) occurs that challenges their abilities to cope. 
 
Transitions’ services include …
  • Individual and family therapy and counseling
  • Psychiatric services
  • Case management and supportive services
  • Crisis intervention services
  • Developmental services for infants and toddlers
  • Special education services
  • Vocational and life skills training services
  • Residential services, including group homes and apartments
Our goal is to help those we serve to reach their potential and to be as independent and self-sufficient as possible.
 
Transitions established the Transitions of Western Illinois Fund with the Community Foundation in 1999 as a way of supporting the agency’s services for perpetuity. Many people served by Transitions lack the funds to pay the full cost of the services they receive. Thus, monies distributed from the fund help support the array of mental health and developmental disabilities services provided by the agency and insure that services will be available to people who need them, regardless of income.
